theres a skill within the skill tree which allows you to block attacks with LB and another skill which allows you to attack with RB straight after the block.
as an enemey attacks use LB, you should see like a gold aura after the block and the enemy stumble, then RB to hit the enemy and do this 50 times and the cheevo is yours

You will need to obtain at least two skills in the constellation map to unlock this achievement. Down from the starting position in the centaur constellation, grab the Cold Blood and Counterattack I skills. This will allow you to time a block with 
right when an attack would hit you for a parry, and if done correctly time will slow down for a moment and if you press 
for an attack during that slowed section, that will perform a proper counter attack for extra damage. You can grab other skills in that same constellation to use a heavy attack with 
instead and increase the damage done, but the basic one works as far as the achievement is concerned.
