I am not sure what caused this to happen, but I have some guidance.
On the level (around 90% through the campaign) you will fight numerous bosses all in one. I managed to unlock this achievement by spamming the big spider boss (Fred) and pushing him into the "rift" which is the moving boundary on this level.
