Not as hard as it seems! As with every mode in this game, there's not really a 'game over' - losing all of your lives simply resets your score and you continue from where you left off.
As the difficulty achievements don't stack I don't recommend trying to slog through this and Nightmare at the same time, so I just did this on Normal.
The five you pick are arguably all as difficult as each other - so take whichever ones sound the simplest to you and obviously avoid the one that does end the game if you die!
