I did it with 2 controllers.
Position your defender on the goal line in the middle. Rush your goalie with Y, shoot a weak shot with your second controller to make sure it's a ground shot and immediately press X on you first controller. Your defender should make a sliding tackle clearance. Please see a clip below.
EDIT: If you are struggling with too much traffic on the defense set the team to 3-4-3 formation and keep fouling till the defenders are sent off. This should clear the path. Credit goes to IRON MAN Ai, see his full comment below.

Right finally did this, got Real Madrid (do not bother, they are extremely tedious, I swear they have yellow/red card unvunerability lol), put them into a 343 formation, got 2 defenders and 2 midfielders sent off clearing a lot of space at the back as only 1 defender was left. If you get a player blocking the shooting 2nd controller player move them into midfield and they will stay there.
Position defender on line, shooting player to the left or right of the D (still inside it though), hold Y to pull keeper and as he approaches the penalty spot double tap shoot on the 2nd controller (I was having problems as the keeper kept saving the shots so keep hold of Y until the shot clears him), and be really fast on the sliding tackle. A few attempts on this because even though he saved them with a sliding tackle it didn't "CLEAR IT UP FIELD" so had to keep doing it. Good luck everyone!

This seems like the best idea. Gonna give it a go... Sick of trying other method. ive had defender clear it with slide tackle, but it didnt unlock coz the attacker won ball back and scored with second attempt.

This is how i did it after struggling trying with 2 controllers and failing.
Set the half's to 20 minutes. Difficulty - Legendary.
Adjust the computers shot error to 1 in the sliders menu.
Turn your player switching to manual.
Play against the computer i used FC Barcelona vs Real Madrid.
Keeps kicking the ball out of play so the computer has a cross.
Position a player on the line, as soon as they take the cross bring the keeper out by holding Y.
If they have a shot on goal tap X to clear.
Rinse and repeat until it unlocks.
Otherwise throw the ball out with the keeper, wait for your player to get tackled, place another player on the line and bring the keeper right out to the computer on the ball.

You can set this up quite easily by playing against the computer on Amateur difficulty, although it isn't going to work every time. Like many other achievements in the game, this is pretty much luck based, so if it doesn't work, keep trying.
Give the opposition a corner and as the ball comes in hold [cn_Y to rush your keeper out. If you opponent gets there first they will knock the ball towards the goal. You can then keep pressing
so that any nearby defender clears.
Sometimes the keeper might get to the ball first, the computer will knock the ball wide or over the bar, or your computer controlled defender might get a touch first, but if you keep trying it will come eventually.
I've managed to recreate it a few times using this method.

This one's not so hard to do vs the computer
1) First of all set sliders

This will ensure your goalkeeper is terrible at saving anything and will make the opposition shoot and pass more accurately without being able to blast it.
Also, change difficulty to LEGENDARY.
2) Now put your formation so 9, yes 9, of your players are in the opposing half. Leave 2 CB's (make sure at least one is fast) and go 'All out Attack'
This will draw your players out the way so the computer is free to shoot at your goal like mad.
3) Now in the game, make yourself a defender and stand on the line. Allow the opposition to come at you and hold cn_T to draw your keeper away, ultimately leaving the goal clear.
The computer should now shoot at goal and you can tap cn_O when the right time comes to clear.
(Note: I actually didn't slide tackle to clear off the line. For me, Ronaldo chipped towards goal and my defender acrobatically cleared off the line, causing the trophy to pop up. This way does work.)
It may take a while, your goalkeeper will be the biggest annoyance but I managed to do it in the first five minutes. Good luck.

Was just playing online friendlies with someone, did the sliding goal line clearance, even got an automatic Upload video recorded called goal line clearance, but the achievement didn't unlock.

This can be tricky so you may need many attempts.
How I did it:
use 2 controllers and give your 2nd control a free kick outside of box. Now move your wall out of way and run keeper out of goal (press the back button to switch to keeper). I ran keeper behind the free kick taker so that he couldn't run back to his goal when I switched back to a player.
Now that the keeper is out of way, move a player onto the line into the middle of goal, using your 2nd pad, hold
and hold up on the left stick, tap 
. This is a " power " free kick but as you tapped it, it should just roll the ball along floor, now with your defender on line, wait for it to get near the line and tap . (you want to aim for a corner to give your defender a chance to slide it out, instead of just kicking away)
If done right and you got the timing right, your player will slide and clear if off the line, it took
me many tries so don't worry if don't get first go.

This achievement is very frustrating and difficult to get if you don’t unlock it naturally. There are four ways that I know of to get this achievement, all of which are quite difficult.
Method One: Two Controllers
Connect a second controller. Turn the passing assistance to manual. Take the ball up with the second controller, once the ball is about 30 yards out bring the goalkeeper out with the first controller with 
. Once the goalkeeper is nearly to the player with the ball, press with the second controller and through ball pass it towards the goal. While this isn’t a legitimate shot, passing it towards the goal will have the ball moving slow enough to make it there in time to clear it. After you’ve passed it towards the goal quickly switch controllers and slide tackle with 
to clear the ball.
Method Two: One Controller
Pass the ball back to your goalie and dribble it out to about midfield. Once you get there, let the ball get stolen. Once it gets stolen quickly switch players to a defender and sprint back to the goal. If you’re quick enough you’ll get there in time to slide tackle with and clear the ball before the CPU scores.
Method Three: Online Boosting
Start a game with a friend who’s willing to help you out. Do the same thing you would do in the Two controllersmethod. Only this method is easier as you can coordinate with your friend and don’t have to worry about switching controllers.
Method Four: Corner Kicks
When the opponent takes a penalty kick hold to bring your goalkeeper out. As the goalkeeper is coming out start spamming . After the opponent heads it, there’s a very slight chance that your defender will slide tackle it right before it goes into the goal, as long as you’re spamming .
