Surprisingly simple.
Select John, and enter a 4v4 heist (can't be training).
Sneak up on unsuspecting AI, and whack them over the melon. Don't assassinate... that doesn't count. If you happen to kill an enemy player in the mix, that counts toward your five, but it's much easier to do it on the AI.
Achievement will pop upon death of the fifth enemy.
