First things first, the majority of the guides using cheese strategies have been patched out. The only way now is to win legitimately. I'll give you a breakdown of ideal spells, items and a location (so other annoying umbral mobs won't spawn) to use.
Other than the achievement and extra umbral scourings you get for a first kill only there is no other reason to attempt this fight until the end game. If you are using an OP build and very good at dodging/blocking you could pull this off mid-game. However the Scarlet Shadows HP pool is massive. It will take several minutes to wear it down if you attempt it. Do yourself a favor and attempt this end game. I personally killed him around level 115.
Tips
- Scarlet Shadow only appears when your umbral multiplier maxes out at x3.00. Takes a few minutes.
- If you go near a vestige or flowerbed he will heal to FULL health
- Cannot use your Sanguinarix to heal. Other options are Brio Stones, Health Regen ring, any rune that gives health back/regen, healing spells/items (see blow)
- The biggest challenge is the overwhelming spawning of other enemies. There are some safe spots you can find usually on stairs/in buildings. I'll list the one I used below.
- It attacks with physical and wither, and it resists all elements and statuses but is weak to physical damage.
- Its attacks are essentially the same as a Bringer of Stillness, just hits harder and has more HP. Can practice fighting them first to learn patterns
- When he goes underground you have a second or two to use a spell/item.
- By now you should have the Crafters Essence from completing Gerlindes questline. Equip to any heavy shield (I used Shield of Whispers). Heavy shields deflect all his attacks then you can just hit him back once or twice to regain health. Rinse and repeat till he is dead.
- Max physical defense and wither regan %/HP from umbral eyes.
Useful Spells/Items
- Odd Stone (Very OP once per life consumable, massively increases physical/holy defense till you die)

- Aura of Tenacity (with a mana regen ring + 1 mana regen rune you can have this up almost the whole fight)
- Any Radiance Healing Spell
- Any AOE spell that will do damage over time as you fight.
- Unripe Berries (Faster Stamina regen)
- Banner/Enhanced Banner Javelin of Protection (Place in the ground and stay near)
Full Strategy (one of many you can do)
- Go to the Windmill Vestige (Marco of the Axe). Go into Umbral and look towards the large bell door. On the left side you’ll see an opening, pull the platform out and drop down to the bottom. Then climb up the ladder on the far side. You will be on this small wooden platform. Do not drop down the other side. (This is one of many areas you can find where mobs do not spawn in)
- As mentioned above, use a heavy shield with the crafters essence rune. Heavy shield will block all his attacks. You can also try to time your dodges if dual wielding but that is more difficult
- Equip umbral eyes that increase wither health gain on hit (Eye of Rosamund and Eye of Bloody Pilgrim)

The longer you stay in Umbral, the more you risk running into this fellow. The Scarlet Shadow will hunt you down and try to kill you. Your aura will start to glow red when he is on your case, and from that point a showdown is imminent.
