This secret achievement (as yet not uncovered on TA - PM when it is so I can remove this part) is called Team Glory.
Unlocked by completing the whole of chapter 3 without dying more than 10 times. It's not too hard, I just seem to have difficulties with Susan sections, but even then I only die once or twice so it's not a hard achievement.
