The end boss of this level has four little eye-like things attached to it, which you can shoot.
In the original R-Type game, shooting all of these eyes caused the boss to become detached from the wires that were holding it in place, and it would advance slowly towards you.
Fortunately, this doesn't happen in this version, however the eyes DO respawn.
I was trying to get this achievement, and I was trying to see if they would stop respawning after so-many times, but I accidentally killed the boss after three times round, and there were still eyes attached to it, but it gave me the achievement anyway.
So I think you only have to make sure that you destroy each eye at least once before you kill the boss.
You can do this in Score Attack mode so that you start off with full power-ups, but it MUST be done in R-Typer difficulty or higher.
