How to unlock the Second Chance achievement in Returnal - Definitive Guide

After completing the first house sequence, in the first map, you'll be rewarded with an Astronaut figurine. Upon dying this will resurect you in the spot you died unlocking the trophy.

The house is in a random room and you house key must spawn with it in order for you to get inside in the first place.

You may also get a random fabricator that allows you to turn in obolites for this figurine, often it's the room with a group of four fabricators attached to a room the has a Xeno translation stone inside.

You’ll need to complete the very first house sequence in the first biome, unlocked by finding a set of house keys out in the world. Completing the first house sequence will unlock the Astronaut figurine. Make sure you collect this artifact outside the house, and check it’s in your inventory. If it is, you’ll find that, the next time you die, you’ll be returned to life. Neat!

The “Astronaut” figurine can be created in the fabricator for oboliths. The house will be opened after defeating the next boss.

There is also an artifact called the Children's Clock, which also works, but upon resurrection it only restores half of the armor.
