This is a bit tricky if you don't know what to do.
Normally, you finish the final boss fight, approach the throne, take the crown, and the game will end. (you will also get the achievement for completing the game).
However, in order to enter the second loop (restarting the game with current cards and stats with increased difficulty) you must not accept the crown in the throne room.
Instead, you must grab one of the large pots found in the throne room, carry it over to the throne, and then drop it on the throne. This will smash it and open up a new portal. Follow the portal and the on-screen prompts and you will get an option to continue to a new loop. Choose this option and the achievement will unlock.
