During your playthough (doesn't matter which game type you select) you may find a corridor with some pots in it.
In some of these corridors, you may notice a stone 'button' on the floor. If you grab one of the pots and drop it on the button, it will open a portal (and unlock the achievement).
These aren't particularly common, but if you see a room with pots in it, be sure to take a look around before smashing them.
I think the first time you see one of these in the game, there will be a 'tutorial arrow' showing you the button and hinting that you should drop a pot on top of it. If you missed it the first time around, no worries. You will have more opportunities.
