Not entirely sure if what I did is the only way to unlock the achievement, so happy for this to be replaced once confirmed.
Relatively early on (approx 1-3 hours) - you receive a side quest from Len in the first settlement where you are tasked to find Mastiff. Once you find him through entering a hole in a cabin behind the house, you climb a ladder to meet a gentleman in the attic called Ninth. Whilst speaking to him, a group of Stalkers surround the house led by Dew. I decided to go outside and assist the man in the attic (Ninth) by killing the other Stalkers and received the achievement.
Apologies about the vagueness, but wanted a solution on here for the meantime.

Did The Ninth die during this fight? It would make sense if you got it from him dying and then you killing Dew's gang. Oddly, I was save scumming that mission and did every possible variation because I wanted to see if there was a way both sides survive and I never got the achievement during the times they killed him and then I killed them

I got this achievement in my first combat encounter after the Prologue. I don't know if that's supposed to happen or not but I killed the three people shooting at the house of a friend of the guy who rescues you. Sorry if that's not helpful, I only played 45 minutes and don't remember his name.
