You'll need to find a corpse wrapped in white binding on the ground when your light meter is almost depleted. It should be on the opposite side of the area where you find SWAT unit corpse to progress.
Once you found the corpse, wait out the death sequence and you'll pop the achievement once you are dead.

This is in Chapter 27. I was hoping it would be in a fixed spawn location from the door, but it's random. From my experience (I've managed to find the location 5-6 times quite fast now that I knew what I was looking for) if you start walking in a direction and the blue bars are immediately going up, it isn't going to be in that direction.
The corpse is visible from a certain distance so you don't have to almost trip on it to find it, it's draped in a white cloth. What did it for me was walking around the edge of the Shadow Wasteland until I had about 5-6 blue bars on the meter at the right of the screen (so about 4-5% of the full bar). Just walk around and keep the light as low as possible (no need to turn the flashlight off or anything) and you should eventually find it. If you hit radiation, you've gone too far.

Here's an example. Notice the light meter, I always try to keep as close to 0 as possible but not quite in the dark. Yours will probably be in a different location but this is more or less how to go about finding the body :
