To unlock this achievement, you need to play as both a Male and a Female, to change your gender go to any Image As Designed. and pay 450$ for cosmetic surgery go to sex and hit female/male whichever sex you need to choose and play for 2 hours!
( EDIT ) this took me much much longer than 2 hours, so don't expect to unlock this straight after 2 hours it may be different from player to player but it took me about 10 hours as a female and 10 hours as a male for cheevo to pop

Is there possibly a bug with this achievement? I unlocked it np on the 360 but 4hrs with one and 4 with the other and still nothing.

i haven't yet got the achievement and i have definitely played for 2 hours on both genders i think the achievement is a bit temperamental but as the game has just released there is going to be some bugs, i suppose we just wait until they hopefully patch this and some other bugs going on!

At the beginning of the game, choose the gender to your liking. Play through the story, or at least 2 hours (2 hours in real time). Once played 2 hours as male or female, go to the Plastic Surgeon ‘Image as Designed’. It’s a face icon on the map. Go inside and press 
. You can change your character from here and also your gender.
You’ll have to pay $500, even if you don’t make any changes. If you buy the place, you’ll get a 10% discount. Also, if you switch gender, you’ll lose all your properties. So either save before changing gender, or even easier, upload your character. (Which will also unlock the "Jumped In" trophy if you haven't done so already.)

You do not have to make different saves for this, just go to Image as Designed and change your sex there. Since you have to play for 30 hours anyway, just change your sex before doing something like finding all the collectibles, and change back later if you're bothered.

If you first played with one gender for 2 hours, and then had plastic surgery and lost another 2 hours with the other gender, but still didn’t get any trophies, there is a solution - change the gender again and get trophies.
