Just like the Elder Dissed achievement, this is only doable after defeating the Dweller of Dread.
To the north of Sleeper Island is a small floating island with a switch you must interact with, which will make light totems rise from the sea.
The totems, one in the North West (slightly north east to this island switch), South West (slightly east of Evermist Island) and East (slightly west from Hikers Rest). You need to light up the switch underneath the water and then follow these lights to a whirlpool for each one.

The Sea Slug is an optional boss you can fight at the end of a side-quest for Resh'an's ultimate weapon. After getting the power of flight you can land on the tiny island in the Northwest section of Home World (labeled "???") and use the glyph there to activate four light prisms in the water around the map.
