The carrier is the boss at the end of level 5. There are two variations.
This would not unlock for me with the Fortress ship, either in a standard run or in Arena. I got it to unlock in Arena by using the Tempest ship.

I've beaten the Carrier with both the Fortress (standard run) and Tempest (Arena), but no achievement. The Carrier I've beaten is the MK-II.
I read elsewhere that it needs to be the MK-I variant of the boss in order for it to unlock. I haven't faced the MK-I yet (that I'm aware of) so will keep plugging away to see if I can get it to appear and see if it unlocks if/when I beat it.

Confirmed: MK-I is the Carrier boss you need to beat for this achievement to pop. I beat MK-II a ton of times with the first three ships, both in a standard run & Arena with no achievement.
