How to unlock the Lazarus achievement in The Binding of Isaac: Rebirth - Definitive Guide
To get this achievement you need to have at least 4 blue or black hearts on you.
Blue hearts are random, are lost first over normal hearts and cannot be refilled.
The easiest way to get the achievement is to do this after you unlock Azazel. Azazel starts with 3 black hearts so if you pick up any blue heart in the game before taking any damage (bosses give them out often) you will unlock him straight away.
Update - I have not fully tried this yet but as achievements for character unlocks can be done with seeds you can get this in the first room if you have Azazel. Use seed 57XRREZ3 and go up from the start, the first room should be a sacrafice room with a soul heart in giving you 4 total soul hearts.

I though seeds voided achievements???

I unlocked this while xbox live was having issues, and now the achievement is stuck saying "Done! Unlocking..." ffs

An alternative method to unlock this is by picking up the PJ item as it gives you 6 blue hearts upon pickup

For this you just need a total of 4 hearts that aren't red (so either the pale blue or black hearts, any combination of them) to appear after your red hearts on your health bar. You must have them all at one time as well. If you have 3 black hearts and get hit, losing a black heart, you'll need to collect 2 more in that same run (without taking more damage) to get the trophy, they do not "stack".
There is an orbital buddy you get who looks like a little black ghost, his description basically says he wants to steal your hearts or something like that. If you're lucky enough to find him, let him pick up every red heart you see drop. He will start turning your red hearts into pale blue ones. If you find him on a run that decides to give you a bunch of red health, you could have this trophy in the first level or so (I got it on Basement 2 after finding said buddy on the first floor). Everything is random so it's a little luck based, but it shouldnt take too long to find the right combo of items.

You can accomplish this a number of ways -
Keep an eye out for and destroying tinted rocks (identified with a 'x' on them)
Buy one for 5 cents in shops if they hold one
Curse Rooms sometimes have Red Chests with one or two Soul Hearts inside
Blue campfires can give Soul Hearts when destroyed with bombs
Angel Rooms can sometimes have two to three Soul Hearts inside them
Items that give Soul Hearts when picked up -
Squeezy - Gives 2 Soul Hearts
The Pact - Gives 2 Soul Hearts (Costs 2 Red Hearts in Devil Room)
The Mark - Gives 1 Soul Heart (Costs 1 Red Heart in Devil Room)
Black Lotus - Gives 1 Soul Heart (Secret Room Pool only)
Cancer (Zodiac) - Gives 3 Soul Hearts
The Soul - Gives 2 Soul Hearts (end game unlock so probably irrelevant)
Super Bandage - Gives 2 Soul Hearts
PJs - Gives 4 Soul Hearts (Afterbirth only)
Binky - Gives 1 Soul Heart (Afterbirth only)

This is best done by bombing Tinted Rocks on early floors. Refer to "Cain" for more info. You can also use the item Guppy's Paw to easily get this one, It is obtained from a devil deal, or by getting lucky inside a curse room which looks like this:
