There are a total of 15 maps; however, from my playthrough only 13 were required for the achievement. The maps you need to collect are:
Ozryn Village (Carcana at The Blue Cauldron)
- Evertonia
- Stock Gauntlet
- Firearm Foundry
- Shimmer Lode
- Snapfrost Dungeon
Nuxvarr Village (Hacim at Hacim’s Emporium)
- Evertonia
- Sky Pavilion
- Old Nux Castle
Dungeons (small chest hidden in the dungeon)
- Stock Gauntlet
- Firearm Foundry
- Shimmer Lode
- Snapfrost Dungeon
- Old Nux Castle
- Sky Pavilion *
- ??? *
The 2 maps with a * beside them were not required for the achievement. Once I collected the map inside the Old Nux Castle (I had all the others), the achievement popped.
The difference between buying a map and finding one in the dungeon itself is that the one you buy in the shop will mark the locations of the weapon parts as ? on the map, whereas the map you find inside the dungeon will clear any of the white fog when looking at the map in the pause menu. So, you do need to find both.

- 5 in the Blue Cauldron shop in Ozryn Village - 3 to start and 2 others that unlock via story progression (clearing dungeons)
- 3 in Hacim's Emporium in Nuxvarr Village - 1 to start and 2 others that unlock via story progression (clearing dungeons)
- 7 in dungons - 1 in each, including the Stock Gauntlet tutorial dungeon. These maps are located in small treasure chests (like the kind that have gems or keys) so make sure you're opening all treasure chests you come across while in dungeons!
Checking the trophy progress tab in the game shows a total of 15/13 towards the trophy so there appears to just be some wiggle room for what maps you collect for this trophy - I unlocked it after collecting all dungeon maps (except for the final dungeon, Varrian Ruins) and buying all but one of the shop maps.
