Achievement description explains what to do: Kill every unique hero once.
There are a total of 88 heroes that you can use. You must kill all of them at least once through multiple playthroughs. The easiest way to do this is to start a new game/save and progress the rooms until you can find a spike or an enemy within a small distance from spawn. Then, all you have to do is just run your hero to the danger and kill them. Once all 88 heroes have been killed, then the achievement will pop.
The other method (but not recommended) is you can pause the game and "deton8 hero!" but this will take up much more time and input.
Getting this achievement will get the related achievements towards death.
