It doesn’t appear to be true that getting all 50 collectibles is needed to unlock those three achievements – for me they were unlocked when I completed (played through) the game for the first time.

There are 13 flowers:

  1. Chapter 3: Carnations - story-related, Hugo automatically walks towards it once he and Amicia enter the small courtyard inside the church.
  2. Chapter 4: Aquilegia - next to the tree in the backyard of alchemist Laurentius's home.
  3. Chapter 5: Gladiolus - on the small hill which oversees the starting area, which can be reached by finding a small path to the left of the main route right before passing the aqueduct and going all the way to the end.
  4. Chapter 7: Hawthorn - inside the furthest house to the right of the well inside the abandoned village.
  5. Chapter 8: St. John's Wort - next to the tree where Amicia and Hugo hang their family necklaces.
  6. Chapter 9: Lavender - behind the second tree to the left of the main path at the starting area.
  7. Chapter 10: Daisy - inside a rat-infested room being shone at by a rotating lantern in the third guarded area in the library.
  8. Chapter 11: Rhododendron - on the castle ground to the right near the outer ring after climbing down the ladder to follow Rodric.
  9. Chapter 12: Cinquefoils - at the end of the path to the left after leaving Hugo's old room in the de Rune estate.
  10. Chapter 13: Daffodil - behind a tree to the right midway along the path after rescuing Hugo.
  11. Chapter 14: Black Nightshade - between some bushes to the further right of the courtyard.
  12. Chapter 15: Rockfoils - at the further side of the storage room adjacent to the starting area.
  13. Chapter 16: Christmas rose - in the small flower field behind the area blocked by a pair of large doors in the ruined city centre where Rodric needs to knock down a door to reach Lucas and Melie. The doors will be opened by a heavy-armor soldier charging into the area to engage the group.

~If missed, can be obtained via chapter select~

There are no flowers in chapter 1 and 2

Chapter 3

Herbarium 1: Carnations - In the churchyard, Hugo will walk up to it to initiate its collection.

Chapter 4

Herbarium 2: Aquilegia - Behind Laurentius's house, by a tree. Keep going around the right until you get there.

Chapter 5

Herbarium 3: Gladiolus - When the way splits and Hugo runs through the aqueduct to the ahead, turn left. A bit uphill and all the way back to where the river. Hugo will come back and follow you to collect his next flower.

Chapter 6 has no flowers

Chapter 7

Herbarium 4:Hawthorn - After the mill, you'll enter an old village. Inside the very last house on the right you'll find it.

Chapter 8

Herbarium 5: St John's Wort - After the cutscene where you hang the necklaces on the tree branches. Hugo is kneeling next to it.

Chapter 9

Herbarium 6: Lavender - Right at the start of the level, behind a tree and a fallen trunk on the left hand side. It’s tucked in fairly well.

Chapter 10

Herbarium 7: Daisy - Inside the room were the rats were. After you've let the rats out (via some lights) to kill the guards, go in and find the daisy.

Chapter 11

Herbarium 8: Rhododendron - On the ground in the outer ring to the right of the one you first come down to. When you are supposed to be looking at the symbol on the ground with Rodric, just ignore him and head over.

Chapter 12

Herbarium 9: Cinquefoil - Immediately after exiting the house, take a left and follow the path round.

Chapter 13

Herbarium 10: Daffodil - After the 'vision' you will find Hugo. After releasing him, look for an opening between the trees on the right. Hugo will walk to it, the sun is shining on it so it’s pretty obvious.

Chapter 14

Herbarium 11: Black Nightshade - Found in the garden, in the back right corner from where you entered, between some hedges.

Chapter 15

Herbarium 12: Rockfoils - At the beginning of the chapter, after going through the door that's closest to Lucas. Take a right.

Chapter 16

Herbarium 13: Christmas Rose - When the guards come looking for you while Rodric is trying to knock the door down, kill them all, then go through that door they came from to find the herbarium at the end of the pathway near some grass.

I noticed one interesting thing. If you see something, but don't know how best to get there, you can just run there. So what if they kill you there? If before your death you still managed to take a collectible item, then when it appears at the control point, it will lie on the map and blink, but it will already be visible. This means that with such self-sacrifice you can quickly take some of the things you need without spending money on ammunition or inventing tactics. This can be easily checked by going to the menu and looking in the list of chapters to see if the items we are looking for have increased by one of the values.
