I'm not even quite sure what the "hold a title..." part even means, but this is so easy to get I popped it on my second ever match in the game, without even knowing there was an achievement for it, or how to put titles on the line. 
All I did was start a normal 1-on-1 exhibition match against a current champion (in the character select screen you can tab over to a list that's just the current champions, if you're not sure who they are.)
I didn't even know the title was on the line, nor how to change that manually, so it must be on by default. I didn't realize that until I won and saw Sting (me) announced as the new FTW champion, and the achievement pop.
