Real simple just load into the match and hold B next to the crowd until you get the barbed wire bat. Took me a couple tries, but I eventually got it from the crowd

You have to be outside the ring and move the left stick to center yourself near the ring apron while holding the right trigger to grab a weapon from under the ring not next to the crowd.

Good tip is the make a custom wrestler whos choice of weapon is a barbed wire bat. He will pull it out straight away.

Building off the previous guide in an attempt to make things a little clearer.
Make sure you’re on the “Exhibition” tab of the main menu. Select the “1-on-1” match type from those listed in front of you, and from the following drop-down menu select the “Lights Out Match” match type.
To ensure you can get this achievement on the first try, use Jon Moxley as your avatar. You can face any opponent, male or female, to unlock this achievement. Once the match has loaded in, inflict some damage on your opponent to allow yourself the space to leave the ring. Once you have left the ring, approach the crowd barrier. Press and hold the Right Trigger (RT) when near the crowd. If using Moxley this will see him retrieve his signature barbed-wire bat.
Approach your opponent and hold down the “X” button. This will see Moxley swing the bat and land on your opponent. The achievement should pop instantly.
