The bottle is located at the base of the Polepole Falls in Choroa Gorge. Unfortunately, there are always three elephants at the falls and they don’t like you encroaching on their turf. To get to the bottle, enter crouch mode (by pressing 
), then skim along the right wall along the rocks till you reach the big boulder at the base of the falls. Here you’ll have to be careful because the elephants are almost surely eying you. When you get the opportunity, go grab the bottle from the center of the basin. Once you grab it, the trophy will unlock. Now, just teleport back to the base camp so you don’t have to deal with the elephants when trying to exit the falls.
