DNA Injector: Apex is a weapon that can be purchased from the Therians faction once you've reached integration level with them. Once you've earned enough favor and bought it from them, all you need to do is equip it in a hero's main weapon slot and confirm the loadout. They'll be automatically "transformed" into an Apex and the achievement will pop.
