Pretty basic achievement. Just complete all the missions. Im not sure what happened with the other guys game but I skipped a few and was able to go back in and complete them. Just make sure you complete them all and the achievement is yours!

I died during one mission and accidentally pressed "Skip Mission" instead of "Checkpoint." That mission was left unchecked in the Replay Mission menu. Even when I played through the mission again (several times), the mission was not marked as completed. Therefore I did not get the achievement.
I don't know if it will pop if I play up through that mission or not, but it looks like the only way to earn the achievement will be to replay the entire campaign.
If you're going through the campaign, take care not to skip any mission, not even accidentally or intending to go back to it.

I missed one mission; Rescue clive, and didn't know I'd skipped it, but after completing the campaign I went back to Replay Mission, quickly did that mission and got the achievement.
