This is unlocked by completing the seventeenth mission in the game, which has you treating Farmer Harris'sick pig.
Go to the pig icon on your map to learn the pig is sick, so your boss will have you go to the dispensary at the Animal Hospital to get some medicine.
Bring the medicine back to the farm and give it to your boss, the achievement will unlock after she treats the pig.
