This is unlocked by completing the eighteenth mission in the game, which has you treating a sick cat.
Go to the cat icon on your map to learn about the sick cat, which you will pick up and take to the hospital. Your boss will end up having you go all the way back to the house you got the cat from to get a stool sample, which you need to bring back to the hospital for testing.
Many trips to the dispensary later the cat will be treated and you will have your achievement.
