I have just popped this 'chieve with my partner after round 8, I died somewhere during the 1st four rounds & the two other guys we we're playing with both died more than once during rounds 5 - 8 (& we actually finished round eight with them both dead. Not just downed - DEAD!).
Therefore the actual specifics regarding this achievement are that a 'team' means you & your partner, not all four people playing as previously stated. A 'map' means one full map consisting of four rounds i.e. 1 - 4, 5 - 8, 9 - 12 or 13 - 16. As they load in a random order this could be any of the four maps - Temple, Aftermath, Crossroads or Zoo.
As for a guide on how to get it - get a good partner, stick together & make sure you revive them if they go down.

I got this last night... all 4 players survived until the start of round 4. All 4 players died during round 4. We continued round 4 and finished it with no deaths. I still got the achievement. So I think as long as you and your partner finish rounds 1-4 (or whatever set you want to attempt) you'll get it. However if either you or your partner is dead at the end of any of those 4 rounds, then you won't.

I believe no matter what this achievement pops for that it is glitched.

This is easier with a few friends covering all angles.Some maps are harder than others but you should have this by the end of the first map (Rounds 1-4) The enemies are easilier in the earlier levels. Some maps are tricky to begin with, but keep at it and with practice you will unlock it in no time.
Pretty much stay together and watch each others back. We used to find and area of the map,and set up camp.
When the Heavy guys appear,1 or 2 provide cover fire while the other 1 or 2 flanks him to shoot him in the back
Instead of wandering off if your running low on ammo do a partner reload.

everythign stated already has it covered with one exception (and this is going only off of what happened with me):
i started a public match with 3 friends starting at round 1. my friends and i never died 1-3 rounds...half way through round 4 a rogue genade caught me by surprize and downed me. my partners were unable to revive me.
that being the case i was confused as to why i got the achievement when the 3 others survived and finished the round. i dont think u have to survive through all 4 rounds. i think u just have to survive up until the 4th round...then complete the 4th round. so if u or ur partner die after accomplishing the first 3 rounds, it might not matter so long as the round ur on (the 4th) is completed.

I believe this is correct, exact same thing happened to me yesterday. It was a team of four, three of us DIED in Round 4, and the last man standing beat the map and we all got the achievement

For info about what extraction is, see "Survival of the Fittest". The 16 rounds are divided into maps, each containing 4 rounds until you move on to a new map. You have to survive all 4 rounds of any one of the maps. 4 out of the 6 maps listed in "King of the Hill" will be included in your extraction game.
