This is a story-related trophy and cannot be missed via normal campaign progression.

In this sequence you must stop the assassination attempt on the Medici family. After you do that, you must find your target and kill him:

All this will take place in Florence, the city you started in. Remember to climb up the view points and open up your map some more. Note that you will fight the target once on the ground but will only make him retreat. You will have a cutscene with Lorenzo (Medici) and then be sent to find the person responsible for the attack. Your target will eventually be on a rooftop that you must climb to be able to assassinate him. There is a small chase once you reach him but it's simple enough.

This is story related and can't be missed. Below is a spoiler do not read it if you do not want to know about the DNA sequence.

You will start the sequence in your stronghold. You will be taught how to upgrade your stronghold and the economic system. Your next target is in Florence, Francesco de' Pazzi. You will meet an informant that will take you to your first tomb. There you will spy on the Pazzi meeting to find out their plan. It turns out their plan is to kill the Medici family. You must protect Lorenzo Medici as their family gets attacked. Once he is in safe keeping within his houses walls you must assassinate Francesco. He is a very easy target though he attempts to run away like a coward. Chase him down and assassinate him when you are close to him. This is an easy DNA sequence.
