How to unlock the Doctor achievement in Assassin's Creed II - Definitive Guide

You have to wait until you get the poison upgrade. Once you do find a guard close to something climbable. Stab him with the poison blade and then climb something next to him, then air assassinate. You can do it by standing on something higher up or by hanging from a wall.

You gain the poison ability in DNA Sequence 5 during the memory ‘A Blade with Bite’. In the beginning of DNA sequence 5 you will unlock the ability to put poison on your hidden blade. Once you are able to do this poison someone. It can be either a guard or citizen it doesn't matter.
