Luck of the moose's guide is very thorough, but i have some things to add/ change.
You MUST be BEHIND the enemy to "assassinate" them (or on top of them if they are prone, basically stab them in the back). think halo reach if you've played it. a slash from the knife from the front is a 2- hit kill i think (although you are usually too dead to swing again).
I dont think you have to hold the button down(i may be wrong, but i have a few dogtags now and didnt consciously hold it down...), and you do not have to first equip then knife the press RT, all you need to do is go behind them and press RB, even if your gun is equipped.
otherwise moose's tips are very helpful!

does this have to be done in the beginning of the game or anytime? i got the russian dog tag, but didnt get this one even tho it my 1st try for a dog tag

I'm not sure what you mean, this is a multiplayer-only achievement, it doesnt matter when you get it in a match

This is a Multiplayer exclusive achievement.
You must stab an enemy from behind to cause a short assassination animation to play. This will cause them to drob their Dog Tag, which you will pick up automatically.
The Game Type Team Deathmatch is certainly the best, in my opinion. This is because you have a greater chance of deploying behind the enemy, making it significantly easier to get a kill with the knife. In my first Team Deathmatch game, while going for this achievement, I was able to get the Melee Ribbon.

To get this trophy you must knife kill another player in multiplayer.
Try to be sneaky and get behind enemy lines and sneak up on someone. Snipers are usually still and not aware of their surrounding so they are easy targets aswell if you don't get discovered.

To obtain a Dog Tag, you have to kill an enemy via knifing by using the 
within melee range. You have to be either behind or coming from the side of the enemy for the animation to take place. This should come naturally through normal gameplay. If having trouble, try flanking enemies (especially snipers) and stab them on the ground.
