From main menu select Blissful Sleep DLC then New Game. Difficulty doesn't matter. To skip intro hold 
for 5 seconds. When you gain control pickup the phone: Emergency Call (Ministry). After conversation interact with phone again select Dial the Number and Call the Euthanasia Center. Select YES. If you finished DLC with good end or never played it you will also unlock:
