This achievement cannot be earned on the same playthrough as the "Crossing Roads" achievement; but also, you will need a prayer from Cleofas to get 100% of the game complete for that respective achievement, so I recommend getting this achievement on your 100% run.
Cleofas can be found in the Mother of Mothers, next to a woman called Socorro. There are several outcomes for this quest, but for the achievement, you will need to acquire the 3 Marks of refuges from the mini bosses atop the Cathedral. The mini bosses are grey and look similar to Esdras who we fought earlier.
After giving Socorro the 3 Marks, Leave the room and Cleofas will move to atop the Cathedral roof overlooking the patio of Silent Steps, however DO NOT TALK TO CLEOFAS IF YOU SEE HIM ATOP THE CATHEDRAL. Doing so currently will prompt his suicide and render this achievement void.
Instead, go back to Albero and speak to the guy inside the church (the guy you talk to through a window). He will give you the 'Cord of the true burying' which you now need to take to Cleofas and give to him. YOU MUST HAVE THE CORD OF TRUE BURYING BEFORE SPEAKING TO HIM ATOP THE CATHEDRAL.
Once you've given him the Cord, he will thank you and relocate himself to the church in Albero. He will give you a prayer 'Tiento to your thorned hairs', and you will get this achievement.

If you are following the Walkthrough written above, you should get this trophy along the way. To get this trophy you have to complete the quest in specific way:

- With the Cord of the True Burying in hand, talk to Cleofás in Archcathedral Rooftops.
- Backtrack to Albero and speak with Cleofás in the church.
This will complete the quest, get you the trophy and the Tiento to Your Thorned Hairs prayer.
You can fail this quest by either:
- Ending Socorro's suffering by killing her (Cleofás will relocate to a cell in the Wall of Holy Prohibitions).
- Talking to Cleofás in Archcathedral Rooftops without the Cord of True Burying in your inventory.
