To get this ending you have to complete

before killing the final boss

You can miss ending B by completing the requirements for ending A before killing the final boss, so it's best to kill him first to get that ending (once you get that ending you'll be returned to your game before you faced him).

Details for completing the ending A requirements are:

First you need to collect the Immaculate Bead rosary bead. It's located in the middle of the elevator system that runs between Albero / Graveyard of the Peaks / Convent of Our Lady of the Charred Visage. It doesn't do anything, but if you have it equipped it will change over your next 3 deaths to:

Bead Marked by Guilt => Beat Tainted by Guilt => Weight of True Guilt

You'll need to have Weight of True Guilt equipped to access the Confessor Dungeons. Each of these dungeons are accessed by the Confessor Statues (7 of each) they are located in:

- Graveyard of the Peaks (where you first pick up the bead)
- Brotherhood of the Silent Sorrow (far left of the map, two floors up from the teleporter)
- Mercy Dreams (entering this area from the top it's the first door you can enter, on the right hand side)
- Grievance Ascends (from the first Prie Dieu go down and right into the next "shaft", it's in the room at the top right of this shaft)
- Library of the Negated Words (going left past Diosdado, the nightwatchman, go straight through and exit the room after him and it's in the top right of the room located after him)
- Mother of Mothers (on the far left of the area, in the bottom right corner of the room below an Archcathedral Rooftops Prie Dieu)
- Wall of the Holy Prohibitions (top left of the area, behind the gate that opens after you beat the boss here, and out the left exit of that room)

When you reach each Confessor Statue, hit it a few times and it will crumble away. There will be a portal left behind that you can enter, but only if you have Weight of True Guilt equipped.

Each "dungeon" is a few waves of enemies one that you need to defeat in succession. It's best leaving this to the end of the game, so you can get ending B first, and at this point they should be really easy with all your upgrades. You could complete all but 1 before you do the B ending, this would mean you still have access to pay off any outstanding guilt too.

Each dungeon you complete upgrades the Thorn that Deogracias gave you at the beginning of the game until it becomes Cvstodia of Sin. There are rewards for the number of dungeons you've completed as well, notably a sword heart and 3x rosary beads that you'll need for the 100%.

Once they're completed you can re-face Escribar and this time you'll be able to reach the top of the Ash Pile, thanks to the upgraded thorn, for the "true" ending.

Though it may be a bit misleading, Ending A is actually the "true" ending of the game for which you have to satisfy a couple of prerequisites, namely completing all of the seven arena battles (see Detestatio Sacrorum). Once you have done this, your 'Thorn' item should have evolved to the 'Cvstodia of Sin'. With this in your inventory beat the last boss (see Summa Blasphemia for more info) and the ending, as well as this trophy, should unlock. You will also unlock the 'Son of the Miracle' aspect (outfit) by getting this ending.

WARNING:

Keep in mind that getting the 'Cvstodia of Sin' will lock you out of Ending B so if you want to get both endings in one playthrough, be sure to get Ending B before finishing all seven arenas!
