How to unlock the Witness of The Miracle achievement in Blasphemous - Definitive Guide

This is a basic list of what you need and what you don't need to get 100% in this game. It can be achieved on NG. On NG+ you can get to 150% due to the DLC and the extra challenges you can add, but I found the difficulty spike off-putting so I started a new NG. Very easy to get if you know what you are doing.

GENERAL

Tips: The above map DOES have the map updates from the recent patch and DLC, so no areas are missing. You need those areas and any items on them. The 3 added are: "Ferrous Tree", "Hall of the Dawning" and "Echoes of Salt". There is only one item in those areas: Fire Enclosed in Enamel in "Echoes of Salt". There is a closed door to the right of "Hall of the Dawning" that you can't open but it doesn't count to the 100% in NG (it counts for the 150% in NG+).

There are 4 very hidden rooms aside from the regular breakable walls:
- 2 Breakable Floors in "The Holy Line" (Both are lower areas on the ground, without enemies and full of rocks. One contains items needed for the 100% achievement while the other is the entrance to a DLC area, but it does count for the 100% map on NG)
- 1 Breakable Floor in "Library of Negated Words" (The bottom floor at the end of the first tall area of the map after the first Prie Dieu. Contains a Bile Vessel)
- 1 Breakable Lower Wall in "The Sleeping Canvasses" (West of the bull painting. Contain artwork from other indie games)

Holy fuck, I thought I'm loosing my mind. All I needed was that indie wall full o paintings. Thanks so much my dude, really appreciated. Got really stressed out about this achievo and you put my mind to rest. May the force be with you and may you long live and prosper, and so on

Stuck at 99.91% and I have everything aside from that one room that Gémino unlocks after completing his quest, though I've heard that doesn't count and you can get this achievement with 99.95% of the map uncovered. So I'm missing one other room and I don't know which!

I would recommend to check out https://www.youtube.com/watch?v=AVkxdH_hDXA, managed to find the last two rooms I needed for the achievement thanks to this video.

Thanks so much for this round-up; turns out I was missing the room with the game paintings (I had 99.95%) completion before I entered that room.

I have this game/achievement on Switch, and I will say that you do not need to do all of these in one run. I did do them all on the same save file, so that probably is a requirement as well if you're not doing all of them in one run.

Needed for 100% completion are the following:

All Health Upgrades (6)
All Fervour Upgrades (6)
All Rosary Beads (32)
Max Rosary Bead Slots (8 slots)
All Prayers (13)
All Sword Hearts (9)
All Bones (44)
All Relics (7)
All Bile Flasks (10)
Ungrade the Mea Culpa to level 7
Buy all Mea Culpa upgrades
Complete all NPC quests (this is implied as most end with a prayer, rosary bead, or other such collectible needed in the above)

Finally, complete the true ending, after having completed the "Nonbeliever" ending first, on the same save. Namely, complete the game once, reload the save, fulfill the requirements to get the true ending, and complete the final boss again
After the final cutscene the trophy should pop for 100%

Here is a great map I highly suggest you import into your image editor of choice and check off each item as you collect it, as there are quite a few.
