How to unlock the I Can Rebuild Her... achievement in BlazBlue: Calamity Trigger - Definitive Guide
Play as Carl in a Versus match, hold D and move Nirvana away from you. Have your opponent damage it until it starts to let off purple sparks, and becomes unusable. After about 10 seconds it should reanimate and just repeat this 3 times. Sometimes it doesn't unlock until the fourth decommission. Cannot be done in Training.

It's not permanently glitched. Try restarting your box since the game can be glitchy with achievements and make sure your setting in Options is Console and not Arcade.

This trophy is titled "I Can Rebuild Her..." and it's for having Nirvana (your doll comrade) broken down 3 times in one round. While it says you have to decommissioned her, it's really just having her break down and stand back up 3 times in one round. You can move her while holding D (X on default controls) and she only takes damage when D is held. Simply move her away from you and let the second controller, foe or your friend beat her while you hold X in order to break her three times. Upon seeing her surrounded by sparks for the third time the trophy should be won.
Go into versus mode with 2 controllers and set the time limit to infinite, Have Player 1 pick Carl and Player 2 pick anyone else. Hold D/ on the first player controller and have the other controller hit with soft attacks so Carl doesn’t get hurt and so Nirvana is taking damage, eventually Nirvana will decommission and you will have to wait for her to recharge her gauge and revive herself. Rinse and Repeat until you get your trophy. This is a lot harder online since you have limited time.
