[STORY] Saw the True Ending. (secret)
To get this you have to first complete at least one story path for each of the main ten characters. Doing so will unlock Hakumen and Nu-13's stories. Complete at least one story path for them will unlock the "True End" chapter. Finish it to see the "True" ending and to get the achievement.

(edit: I originally thought it was necessary to get 100% for each character, but just completing one story path will do. Thanks to Tactical Kurama for the info.)

You actually don't need to get 100%, you just need to complete 1 story path for each character to unlock Hakumen and Nu's stories. Their paths are pretty straight forward though, so once you complete those, you get the True Ending.

Yeah the guide said 100 percent every character I was like FUCK THAT. Glad you just need to beat each characters story (ending doesn't matter)

This achievement is titled "Duh Dun DUNNN!" and it is for seeing the true ending of the story. This is a bit of a pain, given the fact you have to go in to story mode and complete each of the Canon endings with the available characters, then the 2 that are unlocked from doing that and finally complete the new option for the "True End".
There are a variety of paths the stories can go, for each of the stories refer to this link in order to know how you must advance for your Canon Endings...

For the most part it's options in text dialogues, the occasional dimensional drive/Astral Heat finisher (Bottom of the special moves list, uses the gauge) or even winning or losing a fight.

To unlock the True Ending you must first complete every other characters story mode, whether you get a good ending or a bad ending. After you have finished all the characters story modes and the True Ending has unlocked select it and play through its 6 battles. Before playing this make sure your difficulty within the options are set to beginner to make the matches easier on yourself. The last battle will be a best 2 out of 3 round match.
