All you have to do is play as Makoto in her unlimited form. All you need to do is when you select her click the right bumper i believe and then start a fight in versus mode.

Pretty much handed out to you. Unlike the on-disc characters, Makoto starts with her Unlimited form, so you don't have to beat Score Attack first to unlock it.
Go into any mode that allows you to play as Makoto (I chose Versus) and select Unlimited Mode before choosing her, located in the same area of the screen where you pick Beginner Mode. As soon as you enter the fight the achievement will pop. Easy!

The unlimited version of Makoto is included in the DLC so you do NOT have to beat Score Attack with her. Simply go into Versus and trigger unlimited mode by pressing 
before selecting her. The trophy should unlock at the beginning of the match.
