Let me start by saying the Anatomy Lesson achievement is one of the hardest achievements I have ever gotten. It requires a mediocre amount of skill and a lot of luck. I would rather beat all of the Call of Duty's on veteran, all the Halo's on legendary, all the Left 4 Dead 2 campaigns on Hard, and Conspicous gallantry on Medal of Honor before doing this again.

Now that you are tracking that this will be a frustrating, controller breaking (I broke two) achievement let me break it down for you. I am will list all of the injuries and rate them on how difficult they are to get and the best mode to get them on.

The scale is as follows:
1- Annoyingly common and easy to get
2- Easy to get but not as common
3- Takes a well aimed tackle and some luck to get
4- Precision tackling mixed with injuring the same area might get you this injury
5- Incredibly rare, usually involves an Unleash 3-man tackle.

There are two modes that I think are best to use if you don't have a friend to man a second controller for you. The regular campaign will be where you get most of your 1-4 injuries and with luck a couple 5's. Playing through the campaign will also unlock the all the bonus modes you need. The other modes you will be using are prison mode and bonecrusher mode.

Note: In my opinion leg injuries are some of the hardest to cause. My strategy is to sack the quarter back using the dirty dive tackle (X button) from a distance while he is dropping back. Best done on bonecrusher with two players.

Here is the list of all the injuries:

Concussion - 1 Campaign Mode
Easiest way to get this is use your running back to unleash on a defender while pushing up on the Right joystick to hurdle over him. Aim for the head

Skull Fracture - 3 Campaign Mode
Many ways to get this most of them being an Offensive unleash with your RB. I have gotten it with both stiff arm and the hurdle method.
Aim for the head

Pinched Nerve- 2 Campaign Mode
This one may not be severe but it is not very common. Can be gotten with a defensive dirty hit aimed at the head/neck area.

Sprained Neck- 2 Campaign Mode
Dirty hit to the neck area.

Broken Neck - 4 Campaign or Prison Mode
I found the most common way I got this was RB stiff arm unleash on a defender who is tackling from the left side. The animation where the RB turns and uppercuts his jaw.

Shoulder Stinger- 1 Campaign
Dirty hit, aim for the shoulder

Shoulder Dislocation- 2 Campaign or Bone Crusher
Not to hard to get if you dirty hit the should area.

Broken Collar Bone - 1 Prison Mode
Incredibly easy, aim for the shoulder with a dirty hit or unleash. This is the second easiest severe injury to get.

Back Spasms- 1 Campaign
Dirty hit to the back, simple

Spinal Cord Contusion - 2 Prison
Almost shouldn't be a 2 but a lot of injuries that look like they should be a back injury turn out to be an internal organ. Just dirty hit or defensive unleash on the back.

Spinal Fracture - 5 Bonecrusher, if you are lucky Campaign
I only got this injury once. It takes a lot of hits to the back on the same guy and you have to hope it doesn't turn into a spinal contusion. 3 man tackle a RB from the side is your best bet. Good luck with this.

Sprained Knee- 3 Bonecrusher
Use my quarterback sack method mentioned above and aim for the knee cap. Offensive unleash juke works too.

Fractured Knee Cap - 3 Bonecrusher
After a couple knee hits should come naturally use the QB sack method. Offensive unleash juke works too.

Torn MCL - 5 Bonecrusher
Only got this once. Aim for the knee with QB sack and pray, another good method is the Offensive unleash juke.

Torn ACL - 4 Bonecrusher
Not as rare as MCL but still fracture knee cap is more common. QB Sack and pray, offensive unlease juke works too.

Ruptured Achilles - 3 Bonecrusher
Aim for the same foot over and over using QB sack

Severe Ankle Sprain - 1 Bone Crusher and Campaign
QB Sack method towards foot or an Offensive Unleash juke should do it.

Dislocated Ankle - 3 Bone Crusher or Campaign
QB Sack method toward the foot is best.

Bruised Ribs - 1 Campaign
Dirty hit the torso

Broken Ribs - 1 Campaign
Dirty hit the torso

Punctured Lung - 5 Bonecrusher
... I hate this injury. It was the last one I needed. You must repeatedly target the torso with dirty hits to break the ribs first. Then use an unleash 3 man tackle preferably from the front to finish him off.

Bruised Kidney - 2 Campaign
Target the groin area with a dirty hit

Ruptured Spleen - 1 Prison
I literally couldn't go one game without getting this injury. So easy to get, aim a dirty hit towards the groin.

Ruptured Scrotum - 1 Prison
Same as ruptured spleen, just aim for the groin and hope it isn't a spleen.

Broken Humerus - 4 Bonecrusher and Campaign
I have been told by friends who have this game that this is easy to get. Took me forever though so I am giving it a 4 because it is my guide. Aim a dirty hit at the elbow or shoulder area.

Broken Forearm - 1 Bonecrusher
Too easy, aim a dirty hit at the arm/ wrist area.

Fractured Wrist - 1 Prison Mode
Aim a dirty hit at the hand area.

Broken Femur - 4 Bonecrusher
Use the QB sack method aiming at the knee area.

Shattered Tibia - 3 Bonecrusher
Qb sack method at the knee area

Broken Foot - 1 Bonecrusher or Campaign
Qb sack method or offensive unleash juke

That concludes the injury list. Some gameplay advice I have for campaign mode is to have franchise be a RB and LB to cause the best injuries. Focus all of your LB and CB training on strength and tackling first to help cause injuries.

When playing prison mode use the two prison teams. The other teams have players with a high resistance to injury in my experience. Unleash on defense is garbage in this mode as it mostly just causes fumbles.

When playing Bonecrusher use two division three teams. They have a low resistance to injury and it is easy to get sacks. I tried using my campaign team at first against a D3 team and my defense was so amazing they tackled anybody before i could switch players and dirty tackle which was frustrating. Avoid unleash unless going for a 3 man tackle. It mostly just causes fumbles. Never Unleash on 2-point conversions as the injury rate is lower. never unleash when someone is near the sideline or the endzone as once they cross those barriers I have never seen an injury happen.

That is about all I have. Before you ask sorry but no I cannot help you boost, going to Afghanistan in a few weeks so won't be around.

You really had that much trouble with punctured lung? I got it by unleash stiff-arming a guy with my QB. If it helps, the marjority of my QB's stats were stacked (95%+ STR, AGL, TKL, etc...) and I had him on the extra-stamina draining drug (the legal one, can't remember the name.)

I hardly ever pass the ball, i mostly run so i guess that is a good way to get it. thanks for the tip.

Great Guide. You missed 'Fracture Fingers' though. There are 31 total injuries.

Great guide. You can also get ruptured achilles doing a unleashed juke. I only need the spinal fracture injury.

Just so you(s) know, me and my brother got an injury done (counted on the slider) in a 2player versus game, which makes things somewhat easier

There are 31 injuries in the game and some are very rare so when you get the opportunity always take it. The best thing to do is keep tackling until you get a chance to injure someone then choose a body area you haven't tried before. You can check your progress towards this trophy in the trophy section.
