The best way to injure Mexico is to call lots of Zone/Prevent formations on defense. When all the receivers are covered, Mexico will start running the ball the down the field. At this point, press B to switch to the defender closest to him and while holding the left trigger down press A to perform a strong tackle.
When the "Choose the body part to injure" minigame pops up, you know you've done it right; just tap A to inflict the injury. Every injury in Prison Ball knocks the hurt guy out of the game, so once the injury movie finishes playing the achievement will unlock.

In the prison ball campaign level just knock the Quarterback out of the game with a injury

During the Prison Ball game Mike Mexico is the fat bald guy you play against and he is hard as nails and is very very difficult to bring to the floor let alone injure. But fortunately in Prison Ball once you do get him to the floor it is almost always an injury, so good luck.
