Easiest to do on Easy mode, but be careful of charging shotgun enemies. Sometimes you can flank an enemy and hit them before they can turn around. Just watch out for other enemies and you should be fine.

This is very easy and can be unlocked on any Chapter (except prologue). Just run up to the enemy and press 
to melee. It only takes one hit to kill an enemy with melee. Please note that if you are playing on Hard and/or are not using the invincibility glitch you might not get close enough for a melee kill because you might die. Best done on Easy difficulty or using the glitch, either way is fine. If done correctly you will get a blue icon on the bottom right corner of the screen showing “Dental Plan x/10”, which will tell you of your progress.
