The achievement is rewarded for having a carmped inventory (26 items in total).
The following guide is entirely based on the one from Crypt Warden of the SteamCommunity.

This can be achieved in the first chapter.
Here are the steps for getting the achievement and some tips on what do avoid or actually do, to get this:
1) Have 25 items in your inventory NOT including the tranquilizers (which you will only take at the very end - you could take a different object at the end, like the spade or a platypus egg, in fact, this doesn't matter, but in this guide, we choose to take the tranquilizers last).
The 25 items include the dragon of invisibility outside of the gadget shop.
2) Outside of the gadgeteer's shop, interact with the invisibility cloak. You will lose it from your inventory and have 24 items again.
3) Now go pick up the tranquilizers (or any other item you haven't picked up yet). Your inventory is full again with 25 items.
4) Go to the gadgeteer's shop and you will stumble over the Dragon of Invisibility. The game will force you to pick it up and voilà... You'll have 25 items in your inventory and the achievement is yours.
Some tips on getting the items:
* Avoid finishing the gondolier event and irritating Spunky Goal. Otherwise you'll have to finish her event. You need to have access to Baby Goal and Lady Goal. Activate the "rebel hideout" and "platypus off the menu" events by talking to Lady Goal and Baby Goal about how to convince them to have surgery done.

For this trophy, you need to have 26 items in your inventory at the same time. This must be done in Act II, as it's the only part of the game where you can have a bloated inventory.
