To earn a Gold Star Rank, you need to both be above 800k and achieve a 100% purification rate at the same time.
This is easiest achievable (at least imho) in the level: Passion on normal.

Having a Gold Star Rank in a level seems to change a little bit about them. In Passion in the Mirrored City part, I had green vegetable cars instead of those strange purple circuit-board small-fry enemies.
