So I don't want to make this super detailed because you need to proceed on this path to continue the story, and it's fun to experience it on your own. Once you find the cyclops gem, before picking it up, check the room on the other end of the bridge. Kill the two skeleton creatures and pick up the Krell Hammer for the achievement.
If you do pick up the cyclops gem right away you will hear the door to that room shut Just place the gem back on the pedestal to reopen the door and grab the hammer.

The Krell Hammer can be easily missed if you're rushing through the game. To get to its location, you'll need to solve the water puzzle in the castle area. Once you restore the flow of the water to the lower area, a bridge will rise allowing you access to a gem. Instead of picking up the gem, go to the other side of the bridge and through the large door. You'll have to fight two Bone Centipede enemies, but once you beat them, the Krell Hammer will be straight ahead, on some rubble.
If you accidentally pick up the gem, closing the door, you can place is back on its pedestal to reopen the door.

The, Pants, Party
This weapon can be found after solving the water puzzle. You'll come to a bridge with an obvious gem sitting on a pedestal to your right. However, you want to first head left and collect this weapon before grabbing that gem or the door will close. This doesn't lock you out of the achievement as you can always just put the gem back on the pedestal, but it would just waste some time to do so.
