Fear is only available after you earn the Terror Medallion from the Black Star Armors in Stage 4 - See Artifact Collector. Equip Diamond, Inhuman Blade, Princess's Vestment, and an item of your choosing to make this mode easier on yourself. Also, be sure to keep after those stat altering orbs.

On FEAR difficulty you can take 4 items with you. I recommend taking the following with you: Diamond, Inhuman Blade, Princess's Vestment and Aquamarine. Of course, you must first find them and you must have the Philosopher's Stone artifact.
