This achievement can easily be unlocked in the first interlude story mission titled “Love Like Fire” when you are playing as Johnny Silverhand. This mission is story related and cannot be missed no matter what life path you choose at the beginning of the game.
After you get off the helicopter you will reach a set of stairs which you must go down that will have 3 enemies that are very close to each other. You must kill all 3 of them (each after the other) very quickly between kills and at roughly melee weapon range to unlock the achievement. A good place for this outside of story/side missions is a location with three low level enemies in southwestern Arroyo, Santo Domingo (Thanks to Hirsute Dave). The exact location can be seen below:
You must use a powerful revolver (roughly 190 DPS) to be able to kill them in one shot each. Hip firing works best. You can make a quicksave before your attempt in case you fail.

Took me a few tries but I'm pretty sure that was my fault. I don't think I got close enough to one or more of them the first few tries. After I got close enough to all three, worked like a charm!!

Do yourself a favor and save once the person hacking the door to get in the building says Johnny is messed up. Thats the end of the scripted moment and if you mess up you can just load in there rather than sit through them talking a buncha times.

If you are struggling with this, you can knock 3 enemies out line them up, SAVE YOUR GAME and try for 3 headshots. As they are knocked out it should be a one hit kill.
I used non lethal rounds to take them down, finished the area and then lined them up and went for head shots :) good luck

did it work for you on your first try?
i tried it with knockig 3 out (knocked them out with grabbing and non lethal takedown), and after 3 headshots the cheevo didn´t pop for me

You're an absolute legend :) struggled for hours even if my legenadary revolver was my main gun. Achievement done in one minute with my Rocket-arm with non leathel rounds. Just build a stack of 4 knocked out enemys, shot a couple of times, popped immediately <3

(Especially for those way past the early Johnny Silverhand missions)
Many people, including myself, were having trouble with this one. I hope this solution helps and clarifies things.. the main detail to keep in mind is RANGE. You can take out 5 enemies in 4 seconds but if you’re not close enough, this does not work.
1. Get an OP revolver (I was using an Overture with 520 DPS). I’m sure you can use pistols, I just felt revolvers made this easier. I’m not sure how low DPS can be for easy Watson enemies, so feel free to drop some comments if you got this with something less (say 300 DPS?). One shot kills are heavily preferred.
2. Fast travel to Offshore St in Watson. The enemies are here (https://imgur.com/a/hGW5GhM), just down the street southwest from the fast travel point (https://imgur.com/a/PEm05z0).
** This is also a perfect spot to do the Daemon in the Shell achievement too if you haven’t!
3. Make a manual save, it could take a few tries.. or run down the street and come back. They’ll keep respawning.
4. Hipfiring works best for this. Don’t bother aiming. It shouldn’t even matter because of how close you have to be anyways.. Get up close to one. I mean reeeaal close. Like each enemy should have the revolver on their back/stomach. Then take them out one by one. Like I said, focus less on the amount of time it takes to do it, and more that you’re right in these bastards’ faces giving them a kiss of death.

Managed to miss this in the Silverhand missions as well - this is a great spot, thanks 👍 My revolver was 245 DPS. This spot also works well for the “Two Heads, One Bullet” achievement (sniper rifle 222 DPS).

A good place to get this is during the main story mission 'Never Fade Away' - it's one where you play as Johnny Silverhand while searching for a certain woman. You'll know it's coming up when V gets into an ice bath.
After some progression through the mission, you'll be following the woman and there will be a dialogue choice which includes a line about walking her home. At this point, some thugs will approach and more dialogue will happen.
During this, there is an option to pull out your weapon - it's the powerful pistol that Johnny has in most missions. I just walked right into each thug, shot them once in quick succession (there should be three) and earned the achievement pretty easily.
If it doesn't pop, try reloading an autosave quickly - this is the best section for this achievement that I've found in the story so far.

Still works in 1.61, though the revolver didn't work, probably due to the delays between the shots, then a pistol worked just fine.

When playing as Johnny during the story, you'll have a very powerful gun that will insta-kill anyone so you can do this here easily but if you miss this, wait until further in the story so you have powerful pistols then go to a NCPD mission and try to kill 3 enemies quickly with your handguns. You can also use the Request Backup quickhack (bought from Netrunners) to help with this, since all enemies will regroup themselves and will be close together. Just outside of your first appartment, there will be 2 policemen and an enemy on the ground, you can easily do this here but once you kill them they don't seem to respawn so make sure to have a backup save if you attemp that here.

While the achievement description does sound quite straightforward, the requirements are actually pretty strict. Using a revolver or pistol, you need to kill/incapacitate 3 enemies in under 4 seconds while in melee weapon range from each one when you kill them. In the majority of situations where this can be attempted, the time limit is not the issue so much as the distance you are from each enemy you kill. You need to be very close to each enemy, close enough that you'd be able to hit them with a melee weapon equipped. This is what can make this achievement challenging to happen naturally, because most likely at least one enemy is going to back away and move to cover, rather than all of them coming up close to you.
Because of that, the easiest way to unlock this achievement is to find a group of enemies in neutral territory. They can often be found standing around the side of the street, at various locations. I found a group of three enemies very close to each other out front of The Chapel in Pacifica, but anywhere that isn't a hostile area works fine. Since you're not in a hostile area, you can literally walk right up to enemies with your gun out and they don't react. If they're all standing, you may even be able to line up a double headshot, hopefully killing two enemies with a single bullet (as long as the second enemy isn't too far away to not count), and then you can quickly run up to the third and unload your clip into them. Once you do find a group that looks good to try this achievement on, definitely make a manual save before attempting it, so you can reload it and try again if need be (which you probably will need to, since enemies have a tendency to bob and weave when you're trying to shoot them up close).
