This achievement requires you to start a game in Custom mode.
To change the game mode, press cn_back and scroll down to where you see a game mode and BGM. You need to make sure the game mode is Custom. It doesn't matter what settings you use. Once you're done, save your changes.
Once your changes are saved, you can start a game by inserting a coin with cn_Y and pressing cn_start. Select whatever ship, power shot and speed you want, and once the game loads you'll unlock this achievement.
