The goal of this achievement is to finish Stage 1 without the Cyborg counter in the top left of the game hud (not the border stuff) go above the default 1. (The Cyborg counter is the red box under your score and next to the number of ships you have)
Play Stage 1 on Super Easy and don't shoot anything. The boss will eventually timeout and blow up by itself.
Use the save system to play a hit free run. If you get hit and an auto-bomb goes off you might collect some Cyborgs.

What mode did you do this on?
Everytime i start on any mode the counter always starts at 1..

It was probably Super Easy. It always says 1 Cyborg at the start, just ignore it.
