The title suggests this must be done in fever mode but I can confirm it doesn't have to. I got it whilst going for the no hit run in super easy mode. Just utilise the save/load function, I am by no means good at schmups but by the end of the true boss I was around the 6 million mark with 10 lives in hand,each life then gave me a 1 million bonus easily reaching the required score.
