This achievement requires you to obtain an extended item.
An extended item is essentially a one up. While playing through the game, you'll notice on the gadget screen (the left and right sides of the screen displaying all that information) that there is a widget in the middle/right on the right side screen. You'll see a point value and at the end, you'll see the one up image.
Simply get your score up and once you fill that bar, a one up will appear in the middle of the screen and come towards you. Fly into it and you'll unlock this achievement.
